TOWNSHIP ASSISTANCE REQUIREMENTS
BRING THE FOLLOWING ITEMS TO YOUR APPOINTMENT.
FAILURE TO PROVIDE THESE DOCUMENTS COULD RESULT IN THE DELAY OF PROCESSING YOUR APPLICATION AND/OR DENIAL OF ASSISTANCE.
Valid state picture identification for ALL adults in the household (adults are 18 years of age or older) ALL adults are required to sign the application form.
Birth certificates for ALL children – originals no copies
Social security cards for EVERYONE in the household-original no copies
Current copy of your complete lease: include copy of section 8 assistance and utility allotments if applicable or mortgage statement.
Current utility bills including telephone, cellular phones and cable bills
PROOF OF INCOME for the last 60-90 days: such as, pay stubs, child support, AFDC/TANF, unemployment, social security income, short and /or long term disability, letters/checks, tax checks, pensions, annuities, IRA’s, etc.
ALL BILLS/DEBTS, credit cards, student loans, cash advance loans, medical bills, repos, court judgements, bills in bankruptcy, bills in collection, etc.
Receipts for all expenditures the last thirty (30) days.
Vehicle registrations and titles. (Payment book or statement, if applicable.)
Vehicles, renters or homeowners MONTHLY insurance statements.
ENTIRE CHECKING AND SAVING bank statements for the last TWO months and check book roster (bank statements must be current to date of appointment.)
Last TWO employers’ addresses, zip codes and phone numbers WRITTEN DOWN with dates employed and ending wage.
ALL LETTERS/DOCUMENTS regarding food stamps, TANF, short/long disability claims, Social Security Disability claims, bankruptcy, divorce papers, child custody/support orders, doctor statements, etc.
YOU WILL BE REQUIRED TO COMPLETE THE TOWNSHIP ASSISTANCE APPLICATION THE DAY OF YOUR APPOINTMENT.
TOWNSHIP ASSISTANCE APPLICATIONS WILL NOT BE ACCEPTED IF COPIED, EMAILED, MAILED OR FAXED.
